Please fill in the registration form as completely as possible. Once we have received your request, we will review your registration and contact you if necessary. After processing, you will receive a confirmation of your registration by email.
To help us organize care properly, we kindly ask you—if possible—to inform your current GP about your transfer. Your medical record can then be safely transferred to our practice.
