The practice nurse will supervise, advise and monitor you. She will draw up a treatment plan in consultation with you and change your medication if necessary. COLLABORATIONS
Poort van Gaveren General Practice collaborates with several other disciplines in primary and secondary health care.
Pharmacy
As a general practice, we collaborate with several regular pharmacies located near the practice. These pharmacies are listed on our website because we have established good working agreements with them regarding medication safety, communication, and the processing of prescriptions. It is important to emphasize that you, as a patient, are always free to choose your own pharmacy.
After consulting the GP, you can make an appointment with Benoit Schyns (physiotherapy, manual therapy, hand therapy and sports physiotherapy). He carries out consultations and treatments at our practice. It is also possible to make an appointment to see the physiotherapist and the GP at the same time.
After consulting the GP or the practice nurse, you can make an appointment with Yolanda Turksema (dietetics and orthomolecular therapy). These consultations and treatments take place at our practice.
Mens GGZ is an initiative of Stichting ZIO Eerstelijnszorg and Stichting Mondriaan. The aim of this initiative is to support general practitioners (GPs) and their mental health practice nurses (POH-GGZ) in providing care to patients with psychological problems.
Mens GGZ offers solution-focused treatments to people with complaints and problems that require psychological support. We focus not only on the disorder itself, but especially on the person behind the complaints and the questions they bring with them.
We strive for a personal and humane way of working. You can obtain a referral to Mens GGZ through your GP or the POH-GGZ.
The NHG Accreditation and Quality Mark only apply to Poort van Gaveren General Practice. So this quality mark does not apply to the services listed above. These professionals have their own quality marks.
