I am a practice nurse specialising in chronic disease management and graduated 13 years ago. For the past eight years, I have been working in this general practice.
Within the practice, I support patients with chronic conditions such as asthma and COPD. I also provide guidance for smoking cessation and the management of type 2 diabetes (tablet-based treatment). In addition, patients can consult me for preventive care in the field of cardiovascular risk management (CVRM).
I am involved in follow-up care after hospital discharge, for example after a heart attack or stroke, working closely with the GP and medical specialists. I also provide support with the use of weight loss medication, elderly care, and conversations around advance care planning (ACP).
My approach is careful, personal, and well-structured, with a strong focus on collaboration and continuity of care.
In the near future, I will be expanding my role to include practice management. I am currently completing additional training for this position.
